Higher Education Emergency Relief Fund - Student Aid

Information for Students 

The CARES Act Higher Education Emergency Relief Fund-IHE/Student Aid provides funding to institutions to provide emergency financial aid grants to students whose lives have been disrupted, many of whom are facing financial challenges and struggling to make ends meet. Students cannot apply for assistance directly from the U.S. Department of Education but should contact their institutions for further information and guidance. Institutions have the responsibility of determining how grants will be distributed to students, how the amount of each student grant is calculated, and the development of any instructions or directions that are provided to students about the grant.

Formations Institute

Formations Institute acknowledges that the institution has signed and returned to the Department the Certification and Agreement. The school assures that the institution has used no less than 50 percent of the funds received under Section 18004(a)(1) of the CARES Act to provided Emergency Financial Aid Grants to students.

The total amount of funds that Formations Institute has received from the Department pursuant to the institutions’ Certification and Agreement for Emergency Financial Aid Grants to Students is $33,826.00, Part 1 P425E204259.

The total amount of Emergency Financial Aid Grants distributed to students under Section 18004(a)(1) of the CARES Act as of the date of submission is $33,826.00

The estimated total number of students at the institution who are or could be eligible to participate in the Federal student financial aid programs under Section 484 of the Higher Education Act of 1965, as amended, and thus are eligible to receive Emergency Financial Aid Grants to students under Section 18004(a)(1) of the CARES ACT is 29 students.

The total number of students who have received an Emergency Financial Aid Grant to students under Section 18004(a)(1) of the CARES Act is 29 students.

The method used by the institution to determine which students received the Emergency Financial Aid Grants and how much they would receive under Section 18004(a)(1) of the CARES Act is:

“On May 15, 2020 all currently enrolled students will receive a one-time even portion of the HEERF-CARES Act Grant that Formations Institute has received for the students. This grant was applied for and received to disburse to students who where directly affected by Covid-19.

The school’s policy for the disbursement will be that all currently enrolled students on May 15, 2020 will receive their portion of the grant.”

Formations Institute acknowledges that the institution has signed and returned to the Department the Certification and Agreement. Formations Institute  assures that the institution has used the Institutional portion  of the funds received under Section 18004(a)(1) of the CARES Act to provided for Institutional permitted uses only pertaining to laptops, hotspots, audio/video technology, online licensing fees , additional emergency aid to students, employee hazard pay, and other expenses incurred as a direct result of coronavirus/Covid-19.

The total amount of funds that Formations Institute has received from the Department pursuant to the institutions’ Certification and Agreement for Emergency Financial Aid Grants to Institutions is $33,825.00, Part 2 P425F203727.
